These tumors arise from the remnants of the notochord, a flexible, rod-like structure that provides support to the developing embryo.\u00a0 Chordomas are complex tumors to treat due to the involvement of critical structures such as the\u00a0brainstem, spinal cord, and important nerves and arteries. About <strong>30 percent of all chordomas are form within the center of the head in an area called the <a href=\"http:\/\/www.barnaclinic.com\/blog\/orl\/en\/skull-base-tumors\/\" target=\"_blank\" rel=\"noopener noreferrer\">skull base<\/a><\/strong> \u2013 usually in a bone called the\u00a0clivus. <strong>Skull base chordomas<\/strong> are sometimes considered <a href=\"https:\/\/www.barnaclinic.com\/en\/neurosurgery\/brain-tumour\/\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>brain tumors<\/strong><\/a> because they can grow toward the brain.<\/p>\n<p><!--more--><\/p>\n<div id=\"estoy-interesado\" class=\"destacat-site\">\n<h4 style=\"padding-left: 30px;\">IT MAY INTEREST YOU\u2026<\/h4>\n<h4 style=\"padding-left: 30px;\">\u00b7 <a href=\"http:\/\/www.barnaclinic.com\/blog\/orl\/en\/skull-base-tumors\/\" target=\"_blank\" rel=\"noopener noreferrer\">What are skull base tumors?<\/a><\/h4>\n<h4 style=\"padding-left: 30px;\">\u00b7\u00a0<a href=\"https:\/\/www.barnaclinic.com\/en\/neurosurgery\/brain-tumour\/\" target=\"_blank\" rel=\"noopener noreferrer\">Brain tumors: symptoms, diagnosis and treatment<\/a><\/h4>\n<\/div>\n<h2>Chordoma Prevalence<\/h2>\n<p>One in one million people per year is <strong>diagnosed of a chordoma<\/strong>. In gross numbers, <strong>300 patients are diagnosed with chordoma each year in the United States, and about 700 in all of Europe<\/strong>.<\/p>\n<p>Most of the patients are diagnosed in their 50s and 60s, but it can occur at any age. <strong>Skull base chordomas<\/strong> occur more frequently in younger patients, while <strong>spinal chordomas<\/strong> are more common later in life. <strong>Chordoma affects almost the double of men than women<\/strong>. While <strong>chordoma<\/strong> can run in families, this is very rare.<\/p>\n<h2>Probable causes of Skull Base Chordomas<\/h2>\n<p><strong>Chordoma tumors<\/strong> develop from cells of a tissue called the <strong><a href=\"https:\/\/en.wikipedia.org\/wiki\/Notochord\" target=\"_blank\" rel=\"noopener noreferrer\"><em>notochord<\/em><\/a><\/strong>. The <strong>notochord<\/strong> dissolves when the fetus is about 8 weeks old, but some notochord cells remain in the bones of the spine and <a href=\"http:\/\/www.barnaclinic.com\/blog\/orl\/en\/skull-base-tumors\/\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>skull base<\/strong><\/a>. Very rarely, these cells start replicating forming the so called <strong>chordoma<\/strong>. What causes <strong>notochord<\/strong> cells to become cancerous in some people is not fully known, but researchers are working to found the answers.<\/p>\n<h2>Chordoma Classification<\/h2>\n<p><strong>Chordomas<\/strong> al classified based on how they look under de microscope:<\/p>\n<ul>\n<li>\n<h5>Conventional (or classic) chordoma<\/h5>\n<\/li>\n<\/ul>\n<p style=\"padding-left: 30px;\"><strong>Conventional (or classic) chordoma<\/strong> is the <strong>most frequent form of chordoma<\/strong>. It is composed of a distinctive cell type that resembles notochordal cells and can have areas of <strong>chondroid appearance<\/strong>.<\/p>\n<ul>\n<li>\n<h5>Chondroid chordoma<\/h5>\n<\/li>\n<\/ul>\n<p style=\"padding-left: 30px;\">Chondroid chordoma is a term more commonly used in those <strong>chordomas with a chondroid appearance<\/strong>. In the past was difficult to distinguish <strong>conventional chordoma<\/strong> from <strong>chondrosarcoma<\/strong>, but now a days This is no longer a problem because brachyury is expressed in nearly all <strong>conventional chordomas<\/strong>, and other cartilaginous tumors like chondrosarcoma do not express brachyury.<\/p>\n<ul>\n<li>\n<h5>Poorly differentiated chordoma<\/h5>\n<\/li>\n<\/ul>\n<p style=\"padding-left: 30px;\"><strong>Poorly differentiated chordoma<\/strong> is a recently identified subtype. It can be more aggressive and faster growing than <strong>conventional chordoma<\/strong>, and is more common in pediatric and young adult patients, as well as in <strong>skull base and cervical patients<\/strong>. The diagnosis is based primarily because nearly all <strong>poorly differentiated chordomas<\/strong> are INI1-negative, meaning they do not express the INI1 protein.<\/p>\n<ul>\n<li>\n<h5>Dedifferentiated chordoma<\/h5>\n<\/li>\n<\/ul>\n<p style=\"padding-left: 30px;\"><strong>Dedifferentiated chordoma<\/strong> is more aggressive and generally grows faster than the <strong>other types of chordoma<\/strong>, and is more likely to metastasize than <strong>conventional chordoma<\/strong>. It can also have loss of the INI1 gene, but this is not frequent. This <strong>type of chordoma<\/strong> is uncommon, occurring in only about 5 percent of patients, and is more common in pediatric patients.<\/p>\n<h2>Skull Base Chordomas treatment<\/h2>\n<p>Today, the <strong>optimal treatment for skull base chordoma<\/strong>\u00a0is still under debate. The objective is to offer the highest resection index, minimizing intra- and post-operative morbidity and mortality.
