{"id":2204,"date":"2021-07-13T14:18:27","date_gmt":"2021-07-13T14:18:27","guid":{"rendered":"http:\/\/www.barnaclinic.com\/blog\/en\/?p=2204"},"modified":"2021-08-10T09:22:35","modified_gmt":"2021-08-10T09:22:35","slug":"gynaecologic-cancer-surgery","status":"publish","type":"post","link":"https:\/\/www.barnaclinic.com\/blog\/en\/blog\/gynaecologic-cancer-surgery\/","title":{"rendered":"Robot-assisted extraperitoneal gynaecologic cancer surgery has fewer surgical complications"},"content":{"rendered":"<p>In recent years, <a href=\"https:\/\/www.barnaclinic.com\/blog\/blog\/nueva-cirugia-laparoscopica-ginecologia\/\"><strong>minimally invasive surgery<\/strong><\/a>\u00a0has found its way into many operating rooms, and <strong>gynaecologic cancer surgery<\/strong> is no exception. <strong>With <\/strong><strong>robotic surgery<\/strong><strong>, surgical complications are reduced and a better recovery of the patient is achieved. <\/strong>However, there are different possible approaches to perform these interventions. Now, for the first time, <strong>a study explores the differences between all of them in performing para-aortic lymph node dissection. <\/strong>This is a common procedure for gynaecological cancers to see if the cancer cells have spread.<\/p>\n<p><!--more--><\/p>\n<h2>A REVIEW OF THE STELLA-2 TRIAL OF EVALUATION OF THE EXTRAPERITONEAL APPROACH<\/h2>\n<p>This study consisted of a <strong>retrospective review of the STELLA-2 trial<\/strong>, which sought to assess whether an extraperitoneal approach or without violating the peritoneal cavity would be safer for patients undergoing <strong>staging procedures in ovarian and endometrial cancers in early stage<\/strong>. The results of this review have served to conclude that the extraperioteneal robot-assisted approach for <strong>lymphadenectomy<\/strong> or dissection of the lymph nodes arranged around the abdominal aorta, also known by its acronym PALND, was associated with fewer surgical complications. The results of this study have been published in <em>The Journal of Minimally Invasive Gynecology<\/em>.<\/p>\n<blockquote>\n<h4>The results of this recent study have served to conclude that the extraperioteneal robot-assisted approach to lymphadenectomy is associated with fewer surgical complications<\/h4>\n<\/blockquote>\n<h2>A COMPARISON OF FOUR DIFFERENT SURGICAL APPROACHES<\/h2>\n<p>Data from 203 patients from the STELLA-2 trial were included in this outcome review and <strong>served to compare four different approaches<\/strong>. Thus, the results of two laparoscopic approaches were evaluated, one transperitoneal and the other extraperitoneal with another two <strong>robot-assisted<\/strong>, also transperitoneal and extraperitoneal. <em>\u201cIn all cases it was observed that variables such as age, body mass index and waist-hip ratio independently increased the risk of complications in PALND, while the extraperitoneal robotic approach was an independent protective factor for the appearance of these complications&#8221;<\/em>, explains Dr. <a href=\"https:\/\/www.barnaclinic.com\/en\/professionals\/462\/berta\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>Berta D\u00edaz-Feijoo<\/strong><\/a>, <strong>gynecologist oncologist<\/strong> at the <a href=\"https:\/\/www.clinicbarcelona.org\/instituto\/instituto-clinic-de-ginecologia-obstetricia-y-neonatologia\">Instituto Cl\u00ednic de Ginecolog\u00eda, Obstetricia y Neonatolog\u00eda (ICGON)<\/a>\u00a0of the Hospital Cl\u00ednic de Barcelona and\u00a0<a href=\"https:\/\/www.youtube.com\/watch?v=hlNEGtu4nbA\"><strong>gynecologist<\/strong><\/a><a href=\"https:\/\/www.youtube.com\/watch?v=hlNEGtu4nbA\"><strong> oncologist of <\/strong><\/a><a href=\"https:\/\/www.youtube.com\/watch?v=vTBaiin8D5w\"><strong>barnacl\u00ednic+<\/strong><\/a>.<\/p>\n<blockquote>\n<h4><a href=\"https:\/\/www.barnaclinic.com\/es\/cuadro-medico\/462\/berta\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft \" src=\"https:\/\/www.barnaclinic.com\/media\/upload\/cache\/berta_diaz_feijoo_1581353135144_a356356.jpg\" alt=\"Dra.
